Hello,
You are invited to participate in a brief survey about faculty perspectives and behavioral inclinations. The survey includes demographic questions. You may skip any question and may stop at any time without penalty. You must be 18 or older to participate. Participation is voluntary and the survey should take approximately 5 minutes.
There are no foreseeable risks. No contact information will be collected within the survey itself, and responses are anonymous. Participants who complete the survey may choose to enter an optional drawing for a $50 Tango e-gift card. Entry into the drawing is not required to participate in the study. If you choose to enter the drawing, you will be asked to provide an email address through a separate form that is not connected to your survey responses.
Results may be shared in reports, presentations, or publications, but will remain anonymous. The drawing winner will be selected at random and notified by email approximately five weeks after the survey closes.
